We are physicians and other experts dedicated to optimizing the health of those most vulnerable in Central Ohio.
Physicians CareConnection specializes in health and related social needs that directly influence the quality of life for people who are vulnerable. By providing people centered care coordination, our goal is to make connections to resources for people so that they can improve their lives. Established in 1993, by the Columbus Medical Association to enable health care professionals and volunteers to provide quality healthcare for those most in need in our community; PCC has achieved successful healthcare outcomes for high risk populations through our unique model of people centered care coordination.

Statistics
In 2022, we coordinated 4,194 medical appointments for 3,943 people and at least 74% of people helped report NO visits to the Emergency Room. Our reported no show rate for patient appointments in doctors’ offices is 6%. Studies of adult patient appointment keeping indicate that no-show rates of between 15% and 30% in general medicine and urban community health centers are not uncommon.
We processed 1,066 application totaling over $4M in requested rent and utility assistance. We assisted 516 families with $1.9M in rent and utility assistance, preventing their eviction. We completed 848 referrals for social services. This means that people were connected AND received services.
Each year we coordinate at least 1,300 primary care visits. By keeping those visits out of the emergency room, we are saving our community approximately $1M annually. PCC coordinated 2,353 prenatal visits. Over 70% of those visits occurred in the first trimester. Thirty-eight percent of the women connected to early prenatal appointments were African American mothers living in zip codes with the highest infant mortality rates for our county. Our return on investment is 500%. For every $1,000 in investments that we receive, we coordinate $5,000 in medical and social services.
